You already know the gut and the brain talk to each other — we’ve covered that more than once. But here’s what most people miss: the conversation doesn’t start in your gut. It starts in your mouth. The gut-brain axis gets all the attention. But researchers have recently mapped something called the oral-gut-brain axis — a three-stop pathway where bacteria from your mouth travel through your digestive tract, disrupt your gut microbiome, and ultimately affect your brain.
This isn’t theoretical. A 2025 study published in the Journal of Dental Research traced what happens when Porphyromonas gingivalis — the keystone pathogen in chronic gum disease — colonizes the oral cavity. The researchers found that it doesn’t just stay in your mouth. It triggers gut microbiome dysbiosis, damages intestinal barrier integrity, and disrupts a metabolic pathway called kynurenine metabolism.
Why does kynurenine matter? Because kynurenine metabolites directly regulate neuroinflammation and neuronal survival. When that pathway gets disrupted, levels of a neurotoxic metabolite called 3-hydroxykynurenine spike in the blood and hippocampus. That metabolite suppresses a key neuroprotective gene and triggers cell death in the brain — the kind that looks a lot like early Alzheimer’s pathology.
The critical insight: the bacteria don’t even need to cross the blood-brain barrier. They wreck the gut, the gut wrecks the metabolic pathway, and the metabolic pathway wrecks the brain.
Two routes, one problem
There’s also a more direct route. When gum tissue is chronically inflamed, the capillary bed beneath it becomes highly permeable. P. gingivalis produces tiny structures called outer membrane vesicles — essentially bacterial shipping containers that carry virulence factors, including toxic enzymes called gingipains, directly into the bloodstream.
A 2023 review in Critical Reviews in Microbiology documented how these vesicles cross the blood-brain barrier, trigger neuroinflammation, disrupt the brain’s ion regulation, and accelerate the accumulation of amyloid plaques and tau tangles. A landmark 2019 study in Science Advances found P. gingivalis and its gingipains in the brains of Alzheimer’s patients — and the levels of those gingipains correlated with both tau and amyloid pathology.
So you have two distinct pathways converging on the same outcome: oral bacteria contributing to neurodegeneration through both direct bloodstream entry and the indirect oral-gut-brain axis. A 2025 review in Gut Microbes confirmed that microbiome signatures of virulence in the oral-gut-brain axis influence both Parkinson’s disease and cognitive decline pathophysiology.
Why this matters more than you think
