N-acetylcysteine — NAC — has been around for decades. Hospitals use it as the antidote for acetaminophen overdose. Your functional medicine doctor probably recommended it for liver support. Supplement companies sell it as a “glutathione booster” with claims about anti-aging, detoxification, and inflammation reduction.
But here’s what most people miss: NAC doesn’t directly do much of anything in your body. What it does is provide the raw material your body needs to make glutathione — the antioxidant that actually does the work. Think of NAC as the delivery truck and glutathione as the cargo. The truck is useless without the delivery.
A 2023 randomized clinical trial in Journals of Gerontology tested glycine and NAC together (GlyNAC) in older adults and found improvements in glutathione levels, oxidative stress, mitochondrial function, inflammation, insulin resistance, muscle strength, and cognition. That’s a remarkable list. But the study used both glycine and NAC together — not NAC alone. And that distinction matters more than the supplement industry wants you to know.
What glutathione actually does
Glutathione is your body’s most abundant intracellular antioxidant. Every cell in your body contains it. It neutralizes free radicals, recycles other antioxidants like vitamins C and E, supports detoxification in the liver, and plays a role in immune function. Your body already manages oxidative stress through glutathione — it’s been doing this since before supplements existed.
The problem is that glutathione production declines with age. After 40, your levels start dropping. This decline correlates with increased oxidative stress, mitochondrial dysfunction, and chronic low-grade inflammation — all hallmarks of aging. Your cortisol curve is already shifting, your sleep quality is declining, and now your cells’ primary defense system is losing ammunition.
NAC provides cysteine, the rate-limiting amino acid in glutathione synthesis. Without enough cysteine, your body can’t make enough glutathione. NAC is the most bioavailable way to deliver cysteine — better than eating cysteine-rich foods alone, because NAC survives the digestive process more intact.
What the research actually shows
The evidence for NAC is genuinely strong, but it’s important to be specific about what it does and doesn’t do.
Oxidative stress. A 2021 randomized clinical trial in Clinical and Translational Medicine showed that GlyNAC supplementation in older adults improved glutathione deficiency, reduced markers of oxidative stress, and improved mitochondrial function. The participants also showed improvements in insulin resistance, endothelial dysfunction, and cognitive function. This is one of the most comprehensive trials on NAC and aging — and it used NAC paired with glycine, not NAC alone.
Inflammation. A 2021 review in Current Neuropharmacology examined NAC’s role across multiple conditions characterized by oxidative stress — including chronic inflammation, neurodegeneration, and metabolic disorders. The authors found consistent evidence that NAC reduces inflammatory markers, but noted that the effects are most pronounced in people who are already in a state of oxidative stress or glutathione depletion. If your levels are normal, supplementation adds less.
Mitochondrial function. Your mitochondria — the power plants inside every cell — are particularly vulnerable to oxidative damage. The 2023 GlyNAC trial showed improvements in mitochondrial fuel oxidation after supplementation. This translates to better energy production at the cellular level. If you’ve been experiencing unexplained fatigue or brain fog after 40, mitochondrial dysfunction could be part of the picture.
Liver support. NAC’s most established clinical use is liver protection. It’s the standard treatment for acetaminophen overdose because it replenishes glutathione in the liver faster than any other intervention. For people with fatty liver or metabolic concerns, NAC has a legitimate role in supporting hepatic detoxification pathways.
Why glycine matters more than you think
Here’s the part the supplement industry glosses over: NAC alone may not be enough. The landmark trials showing the most impressive results all used GlyNAC — glycine plus NAC together. There’s a reason for this.
Glutathione is a tripeptide made from three amino acids: cysteine, glycine, and glutamate. NAC provides the cysteine. But if you’re also low on glycine — and many people over 40 are, because glycine production declines with age and most diets are low in glycine-rich foods like collagen and bone broth — then supplementing with NAC alone only solves one-third of the supply problem.
A 2023 review in Ageing Research Reviews highlighted glutathione’s central role in both biochemistry and successful aging. The authors noted that maintaining adequate glutathione requires not just cysteine availability but also sufficient glycine and the enzymatic machinery to assemble them. Taking NAC without addressing glycine status is like having the engine parts but missing the mechanic.
Practical takeaway: if you’re supplementing with NAC, consider adding glycine. 2-3 grams of NAC with 2-3 grams of glycine daily is the dose range used in the clinical trials. You can get glycine from collagen powder, bone broth, or a standalone glycine supplement.
What NAC doesn’t do
Let’s be clear about the limitations.
It’s not a cure for aging. NAC supports glutathione production, which supports cellular health. That’s not the same as reversing aging. The improvements in the clinical trials were real but modest — better markers, not miraculous transformation. Your cells are aging whether you take NAC or not. NAC just gives them better tools to cope.
It doesn’t work if you’re already replete. The biggest improvements in the trials were in older adults with documented glutathione deficiency. If you’re 45, eat well, sleep enough, and have no oxidative stress markers, NAC supplementation may produce minimal measurable benefit. The people who benefit most are those who are already depleted — which, admittedly, includes a lot of people over 40.
It’s not a replacement for fundamentals. NAC can’t fix a diet low in sulfur-rich foods (garlic, onions, cruciferous vegetables), chronic sleep deprivation, or unmanaged stress. Your cortisol is doing more cellular damage than any supplement can repair. NAC works best as a complement to the basics, not a substitute for them.
What we still don’t know
The long-term effects of NAC supplementation in healthy adults over 40 haven’t been studied in the kind of large, multi-year trials that would give us definitive answers. Most of the impressive data comes from smaller trials in people with specific deficiency states or chronic conditions. Whether a healthy 50-year-old benefits meaningfully from daily NAC over 5 years versus simply eating more sulfur-rich foods and maintaining adequate glycine intake — that question remains open. A 2024 review in Cureus on NAC’s clinical applications noted that while the safety profile is excellent and the mechanisms are well-understood, large-scale outcome data in aging populations is still lacking. The science is promising. The evidence for long-term supplementation in healthy people is still developing.
